Those two men you see in the image are called Soyjacks. These Soyjacks are created by tracing over a real life man or cartoon making a silly expression, often to mock said person or cartoon (You ever saw a meme consting of two people pointing at something in the background with a surprised expression? That originally came from a Soyjack of two men reacting to vegan chicken nuggets)
While these Soyjacks were originally created by rightist circles such as 4chan and such, they've… changed, a bit. Some people started growing attached to the cartoons and made various edits of the soyjacks in different, weird situations such as dancing, or soyjack edits that seem to be parodying something that doesn't exist, or soyjacks with fruit in their heads. One of such edits is the picture you see of two soyjacks just kind of hanging out with eachother, with no offensive background that I know of.
So the intention behind this is really hard to identify. While it most likely is one of the alt right groups using the soyjack's mocking nature to make fun of this person, it could also be a really, really weird and slightly insensitive way to express awareness or even sympathy. The people who made the weird soyjack edits don't necessarily do it out of hate for someone else, sometimes its just for the fun of seeing these soyjacks do weird things.
