They can't completely control everything, even with a chain you can still move around, pull at the chain and brake it, they cant keep eyes on literally everything. Add infighting between them and the whole thing starts to crack. Majority of control comes from perception of control, basically they want people to think that they control everything since its easier to control things that way.
As for why are we finding out the awnser is same as above, they cant keep an eye on everything and thing accumulate, now they are in damage control and are trying to salvage the situation.
