The way I came to understand it is that the way treatment for gender dysphoria works (at least in ideal, unrushed circumstances), is that traditional therapy is always the first step, and only when that fails are further steps taken. It's why you get trannies that run the gamut of just growing their hair out and wearing a dress, to lasering their facial/body hair and getting HRT, to the full-on dick chop; each of those are basically stages of reducing the severity of the depression their own body brings them. Not all trannies need to go all the way to feel comfortable. I didn't say it was IDEAL, but short of somehow transplanting their brain into a different body, as of now it's the best we can do to reduce their suffering for severe cases, at least without destroying who they are as a person with something like a lobotomy.
