The Folllowers always have been sort of wimpy and dependant on the player… I imagine that all the VD did was leave a power vacuum that eventually got filled by outsider gangs as time passed and the NCR never bothered to try and "colonize" it, so it went back to being like usual.

I'd imagine it changed names and identity as time passed after becoming interconnected with the other "cities", becoming part of NCR as a whole as we know it today.

Caeser’s Legion is probably the largest “nation” in terms of surface area, encompassing all of New Mexico and Arizona, most of Colorado and parts of Southern Utah.
NCR takes the overwhelming majority of California, except the North part and San Francisco Bay Area. A little bit of Northwestern Nevada, up to New Reno and Vault City, which is, debatably, under their control, and the northern half of Baja California.
The third largest would be the Midwestern Brotherhood of Steel from Tactics, if you consider them canon, which I do. They control territory from Chicago, Illinois, all the way to Colorado , but I am sure that they would have been forced out of Colorado by The Legion.
